Managed Care Programs


Before you can enroll into a managed care program, you must be eligible and apply for services.

  • Healthy Options
    (HO) is a medical program that provides no cost healthcare services for people on Medicaid or receiving Temporary Assistance for Needy Families (TANF). See the Service Map for availability in your county.

  • Washington Basic Health
    (BH) and (BH+) is a state-sponsored medical program for Washington residents.  This program requires premium payments based on income and is available statewide. See the Service Map for availability in your county.

  • Patient Review and Coordination
    (PRC) is a program only for clients on Restriction programs.  If you have been placed on restriction, this program will manage your care for you.


  • Chronic Care Management
    (CCM) is a program for aged, blind, and disabled or SSI eligible clients without Long Term Care (LTC) and/or Medicare. This statewide program provides care management services to help clients live healthier lives and prevent health conditions from getting more serious.
