GAU MANAGED CARE PROVIDER INFORMATION

The Community Health Plan (CHP) covers managed care benefits as defined in GAU Managed Care Benefits, for all 39 counties in Washington State for GAU recipients. The Welcome to Washington State's GAU Managed Care Program presentation will assist providers in this transition. After enrolled, GAU clients will receive a Services ID Card and a CHP card to show providers so that eligibility can be verified. Providers and claims are handled as managed care. One exception to billing CHP is hospital UB92 and DRG claims. These claims are to be billed to DSHS. Professional claims for hospitalizations and all else are sent to CHP. For hospital services, use the DSHS client identification (not CHP's), and the DSHS provider number (not CHP's) when billing.

GAU Clinics : These are the primary care clinics clients may choose for their primary care provider (PCP). If a client does not chose a clinic, the client will be assigned to a clinic. GAU clients may change their clinic for the next month if needed by calling CHP at 1-800-440-1561 Monday-Friday, between 8 AM and 6 PM. For information about CHP, go to www.chpw.org

GAU DSHS Evaluation Forms : The physical evaluation (13-021) and the psychiatric/psychological evaluation (13-021a) forms are to be completed by the medical provider so the Community Service Organization (CSO) worker can determine if the person is eligible for GAU. This is paid for by the CSO.
