Processing a Limitation Extension Request: Effective immediately, please forward all Limitation Extension requests to fax number 360-664-4371 (Attn:  First Steps Program Manager).  Form 13-884 (Limitation Extension Request – Maternity Support Services and/or Infant Case Management) has been updated to reflect this new number.  Use this link to find all MSS/ICM forms.
