REFERRALS TO THE DIVISION OF VOCATIONAL REHABILITATION (DVR)
The General Assistance-Unemployable (GA-U) managed care program includes a process to refer those who are able to participate to DVR. Individuals with significant barriers to employment due to physical and mental disabilities may be referred to DVR to prepare for, obtain, and retain employment. DVR also assists individuals with long-range career goals. The Care Coordinators from CHP will complete a referral process so a client can begin readiness for employment.
Referral Process:
Care Coordinator assesses client’s readiness for DVR referral:
- The individual must want to work.
- The individual must require vocational rehabilitation services to eliminate or reduce barriers due to incapacity.
- The individual must agree to the referral to DVR and be willing to fully participate in DVR’s Individualized Plan for Employment (IPE).
- Some individuals may be considered for supported work programs or other activities prior to a referral to DVR.
- The individual who appears to meet DVR’s eligibility criteria will be given the choice of a referral to DVR or other appropriate activity.
- If the individual voluntarily chooses to be referred to DVR, they will be held accountable for the reporting of participation in the DVR process as part of their IRP or GA Case Plan.
Documents the Care Coordinator will assist the client in completing for a referral packet to DVR to schedule an orientation with client:
- Application for Vocational Rehabilitation Services
- Request for Records
- Authorization
- Notice of Privacy Practices For Client Confidential Information
- Consent
- Consent For Private Health Information Addendum
Care Coordinator sends referral packet including release of information to DVR Liaison along with the following information:
- Medical Documentation with diagnosis of condition (from PCP or consulting psychiatrist)
- Copies of the medical record to support diagnosis
- Any notes that the client and Care Coordinator have discussed readiness to go to work
- Assess Chemical Dependency: Client should be clean and sober at time of referral
Care Coordinators must communicate to DVR the need to include CSD staff. DVR has assigned liaisons for each CSD Community Services Office (CSO).
