Referrals for SSI/SSA Disability Facilitation (GAX)
The maximum period of eligibility for GAU is 12 months before incapacity status must be reviewed with additional medical evidence. The evidence accumulated while an individual is a part of the GAU Managed Care Program through CHP is valuable for these redeterminations. Medical evidence and the expected length of time before a client is capable of gainful employment decide when an individual’s GAU will end.
Cases that have received GAU for 12 months or more, or appear to meet SSA Disability Criteria will have their program changed to GAX and no longer be eligible for managed care. The individual will be eligible for Medicaid. They will receive the assistance of a Social Worker at their local CSO to apply for social security benefits, which includes:
- Assessing client needs and appropriateness for SSI facilitation.
- Helping complete SSA application forms.
- Tracking SSA applications.
- Coordinating services
Care Coordinators may also make referrals for GAX by using the Community Health Plan of Washington Disability Screening Tool. The completed referrals are then sent to the following Regional Representatives:
