Renewing Your Child's Medical Benefits

Each year, families with children on medical assistance must verify their income to be sure they still qualify for state medical programs. You should receive the Family and Children's Medical Benefits Renewal form in the mail.

You can process your yearly renewal by completing one of the following options:

By phone: Call the phone number listed on the cover letter sent with your Eligibility Review.
By mail: The Eligibility Review you received in the mail or the new simplified Benefits Renewal from the Internet at http://www1.dshs.wa.gov/word/ms/forms/13_836.doc.

Return your completed review or renewal form to your local Community Service Office (CSO) or mail to:

           Department of Social and Health Services
           P.O. Box 45531
           Olympia, WA 98504-5531

Act quickly. Delay may cause a break in coverage.
